Treating the Jawline and Chin using Dermal Fillers

As an experienced Aesthetic Nurse Consultant I usually find that people are still wary of considering dermal fillers to enhance, define and naturally contour facial flaws. Typically due to negative media coverage, celebrities and public figures who may have gone “too far’ and furthermore lack of knowledge on how dermal fillers integrate with our skin tissue.

Statement about Restylane from manufacturer,

“In the late 1980s, Swedish scientist and entrepreneur Bengt Ågerup had a vision to bring his scientific work on HA out of the lab and into the real world. Ågerup and his fellow scientists wanted to find a way to restore skin moisture and structure by creating a product that could deliver naturally occurring HA into the skin. It had to be safe – closely resembling natural HA so that it would not be detected by the body as a foreign substance and cause an immune reaction – and offer long-lasting results4.

Typically dermal fillers may be best known for improving fullness of lips and cheeks, but recently there has been a huge surge in enhancing and shaping the chin and jawline. These techniques can really transform the overall look of the face by balancing and harmonising features. I believe the reason for increased demand is that clients are moving away from surgical intervention, the reality is for anyone having surgery they will still need to use dermal fillers, muscle relaxants and skin rejuvenation treatments. Surgery will not address ageing issues forever.

Over the years I have been able to successfully use dermal fillers to the neck, face, hands and décolletage to treat the signs of ageing. Over time collagen and elastin fibres in the dermis of the skin break down, muscles start to lose tone and the face begins to sag with results in a jowl appearance and is directly associated with a typical sign of ageing. In conjunction the chin starts to wrinkle and has a dimpled appearance. This area of the face is perfectly suitable for dermal filler to help smooth and redefine the chin to give it a sharper more youthful look.

In recent times younger patients who have a weak chin or less defined jawline will benefit due to facial asymmetry or dental Alignment issues, so although they do not require treatment for ageing they can create better facial balance.

Four Point Face Lift

This month I will be looking at how a non surgical Facelift can be achieved with dermal fillers, in recent years the demand for non surgical treatments has grown. I find that clients sometimes fall into the category where they are not old enough or not ready for a surgical approach. The beauty of using dermal fillers to lift the face is that you can go slowly, building up the areas and you have more control over the final outcome. Also they wear off over time and can be used in conjunction with dermal roller, laser or hifu to maximise results and increase the collagen and elastin to the skin tissues.

THE FOUR POINT FACELIFT

The four point Facelift uses the main areas of the lower face which over time sadly sags and shows volume loss. Firstly lifting the face by augmenting the cheek area is a must, this area is like the shelf of the face, if it is not stable, the lower regions will not lift fully. Secondly I address the nose to mouth lines, I think of this as the supporting scaffold, mid face rejuvenation and restoring the balance of the ‘golden triangle’ will automatically make a face look more youthful. However , one of the most troublesome and ageing features for a person are the ‘dreaded jowls’ or heavy jawline. Personally I like to use a sturdy dermal filler along the jawline to lift the jowls Aswell as lifting the mandible erotically next to the ear area. It gives a natural shaping and definition to the lower face and can help make the neck look less saggy. I tend to choose different types of dermal or volumising fillers, in accordance with age, fat loss and skin integrity.

Treatment Choices for Youthful Eyes

This month I will discuss and look at ways to help tired eyes, and the reasons as to why your eyes look tired, old or dark. When my clients attend for a consultation or new treatment, the eye area is normally always an issue in some way, the eyes are the windows of our soul, when you meet a person you naturally look at their eyes. Most clients seeking treatments will say ‘I’m sick of people saying I look tired!’ These comments damage a person’s confidence when constantly repeated. There are fantastic cosmetic and make up applications that can help disguise imperfections, but without a doubt it is harder to conceal problematic issues around the eye area.

Few of us escape the occasional raccoon-eye when we’ve not slept well or been unwell with an illness. This is a temporary condition solved with rest and improved health. But what’s going on if you ALWAYS look in the mirror and see that “raccoon” looking back at you?

Here are some suggestions of non surgical treatments that can help with specific issues. The right mix of these nonsurgical procedures can nearly mimic the results of plastic surgery with far less risk and downtime.

Dermal fillers – Volume loss directly under the eye is the top culprit in creating under-eye hollows. Light doesn’t bounce off your facial contours equally. Projected areas get more “bounce” – cheeks, lips, nose, forehead…Any concave area is left in the shadows, so they look darker. Such is the case with loss of volume under the eye. So by using droplets of Hyaluronic acid fillers which draw water to the sugar derivative particles helps to re-volumise the hollow ring around the eye naturally.
Furthermore, Volume loss under the eye and in the upper portion of the check will dramatically accentuate bags since the “bag” is now the portion that gets the most light reflection. Subtle cheek enhancement will help to reduce the sagging folds under the eye and restore facial balance. I use Hyaluronic acid fillers or calcium based fillers to give definition. However if there is a lot of dystrophy or facial sagging then poly lactic based fillers are great and almost give a face lift effect!

Plexr – Eyelid ptosis is simply loose skin; a result of age and sun exposure. Your eyes look smaller when the skin above and under the eye is loose and wrinkled. Again this appearance is accentuated by the light/shadow interplay of wrinkles. Plexr is a plasma based technology which emits an electrical arc and penetrates the targeted skin surface and will tighten eyelid skin by shrinking and lifting the tissue and spurring new collagen production for a renewed, fresh appearance of the skin with less wrinkling and smoother skin surface, it can also be used around the crows feet wrinkles and under eye crepey loose skin.

Kissable, sensuous, youthful and plump lips appear to be the key words clients use when referring to how they would like their lip and mouth area to look.Therefore, It is essential to understand how the lip area ages and why certain structural changes occur. The lip and mouth area is exercised daily, talking, eating, smoking & chewing gum all work the muscles around the top lip known as orbicularis oculi and on the lower aspect the depressor anguli oris. Because of the muscle working hard, we see over time that in some individuals prominent vertical lines appear across the upper lip radiating from the base of the nose often referred to as ‘smokers lines’ regardless if you have smoked or not! These lines can span across the lower lip also.

In conjunction to this, the ageing process is responsible for the loss of the vermillion border which runs along the edge of the upper and lower lip. This gives the mouth a sharper, more defined outline and stops the dreaded lip stick bleed. In relation to treatment options, dermal fillers can really help to smooth out these vertical lines by plumping up the collagen depleted area. Plexr which is a plasma emission application, can help lessen the depth of the creases and smooth out the finer lines by lifting, shrinking and tightening the skin tissue. The edge of the lips is greatly rejuvenated with small amounts of filler, remember you don’t need to have bigger lips, the majority of lip clients just have the outline, which gives a youthful natural result.

When it comes to addressing the pull or sagging of the bottom lip, or the ‘cheer up love’ lines, again dermal filler placed in this area gives support and stops the corners of your mouth from drooping. Your eyes should be your first feature to stand out on your face and then the mouth, if it’s the opposite way around you have gone too far and made the face imbalanced.

1 The science – How does it work?

This time of year we can feel the excitement building to Christmas, with parties to attend and busy shopping trips, it can leave us feeling and looking exhausted! Here are some tips and ideas to help alleviate some stress.

EAT WELL – It can be tempting to snack on foods high in sugar when the weather is colder and we want a quick energy boost. Unfortunately this will lead to increasing blood sugar levels.Remember when our levels suddenly dip again, we’re left craving another sugary pick-me-up or stimulant such as caffeine.

– Try to stick to regular meals containing healthy proteins, such as oily fish, grass-fed meats or lentils. If you feel hungry between meals reach for protein rich snacks such as homemade energy balls, houmous and vegetable sticks or simply some dried fruits and nuts.

TIRED WASHED OUT COMPLEXION – All the extra stress, both at work and at home, inevitably takes its toll on one’s appearance. Lifeless dull skin, dark shadows under the eyes become even more apparent. Common symptoms including spots, psoriasis, eczema, dry skin and, if you’re
already run down, cold sores, dandruff, overtly greasy or dry hair — and even hair loss can all
affect you badly at this time of year.

STRESS AND EXHAUSTION – the temptation to reach for a pick me up in the form of a mini

Why does stress affect our skin so badly? Firstly, hormones. Being under pressure increases the production of cortisol, one of the functions of which is to suppress the immune system (that’s why we use hydrocortisone to stop allergic reactions).

– Cortisol can also lead to skin ageing, as it weakens the elastin and collagen structures within the skin. Hence that dull, lacklustre appearance that you get when you’ve had a succession of late nights in a row.

Hormones, too, are the culprit in stress-related hair loss. This used to be a mostly male affliction; but since we women decided, quite reasonably, that it was time for us to have a go at running things, more and more women have started losing their hair through stress. Stress results in waves of androgens, males hormones that have a deleterious effect on our hair follicles and shorten their life-cycle, initially causing thinning and eventually, over time, the death of the follicle.
-Try Aromatherapy oil baths to soothe the body and mind, massage, reflexology, reiki and
meditation can all help you to recharge.

This month I am going to focus of some safe sun tips and facts, most of you will no doubt be heading off for a sun filled holiday , whilst those of us with children are especially good in choosing the right creams, SPF swimsuits and all in one suits, hats , uv tents and brollys. The same can’t be said for our own concern on being safe in the sun and avoidance of sunburn. Also addressing some body concerns that when are bodies are more exposed on the beach we are more conscious off and how to remove them quickly and effectively before the holiday season.

Skin tags are small flesh-coloured or pigmented growths that hang off the skin and look a bit like warts. They’re very common,harmless, and vary in size from a few millimetres up to 5 cm wide!.
They’re usually found on the neck, armpits, around the groin, I new thighs or under the breasts. They can also grow on the eyelids or under the folds of the buttocks.

Why skin tags occur
Anyone can develop skin tags, but they’re particularly common in older people and people with diabetes. Pregnant women may be more likely to develop skin tags, caused by changes in their hormone levels. Some people develop them for no apparent reason.
Skin tags tend to grow where skin rubs against skin or clothing. This would explain why they also tend to affect overweight people or younger children who have excess folds of skin.
Removing skin tags can be effectively achieved by using cryotherapy (freezing off). This simple methods takes a few seconds, mild discomfort at site, normally drop off witching a week or so depending on the size of the skin tag.

Milia A milium cyst is small, white bump that typically appears on the nose and cheeks. These cysts are often found in groups, and in these cases are called milia. The cysts occur when keratin becomes trapped beneath the surface of the skin. Keratin is a strong protein that is typically found in skin tissues, hair, and nail cells.

Milia can occur in people of all ages, but they’re most common in newborns. They’re typically found on the face, eyelids, and

What Causes Milia?
The cause of milia in newborns is unknown. It’s often mistaken for baby acne, which is triggered by hormones from the mother.

In older children and adults, milia are typically associated with some type of damage to the skin, such as:

-blistering due to a skin condition
-burns
blistering injuries, such as poison ivy
-skin resurfacing procedures, such as dermabrasion or laser resurfacing
-long-term use of steroid creams
-long-term sun damage

Treatments include:using a sterile needle to pick out the contents of the cyst
medications, such as topical retinoids (creams that contain vitamin A compounds)
chemical peels
laser ablation, which involves using a small and focused laser to destroy the cyst
diathermy, which involves using extreme heat to destroy the cysts
destruction curettage, which involves surgical scraping and cauterization to destroy the cysts
cryotherapy, which involves freezing and is the most frequently used method to destroy the cysts

SAFE IN THE SUN TIPS

FACT – According to the British Association of Dermatologists a survey carried out showed that 8 out of 10 people are failing to adequately apply sunscreen before going out in the sun.. Furthermore, 80% dont app,y sunscreen before going out in the sun or shortly afterwards. Shockingly only 35% of people surveyed would only seek shade if they were hot rather than to avoid burning! There is no doubt that health campaigns have helped in making people aware of the link between sunburn and skin cancer , but there is a great need to educate on the right sun cream and how to properly apply and reapply.

FACT – Men’s skin cancer knowledge lags behinds women’s, according to the 2016 American Academy of Dermatology survey only 56% of men knew that skin cancer can occur on areas of the skin not typically exposed to the sun, compared with 65% of women. It is so important to regularly check moles and be aware of changes or new marks or lesions to the body. Get your partner or friend to check your back and scalp as its obviously a tricky area to scan yourself. Alternatively there are many private clinics that do full body mole screening.

FACT – if sun creams feel too heavy and greasy on the face and body we are less likely to want to use them!
